Lakeridge HealthWhitbyDUTIES AND RESPONSIBILITIES :
The Community Transition Liaison is the first contact for clients. The successful candidate will triage and coordinate referrals to other services where appropriate, provide support and preliminary education to family members, partners, and significant others, maintain clinical records and work effectively with community partners.
As an integral member of the Positive Care Clinic, the Community Transition Liaison is responsible for providing a wide range of clerical duties including; booking appointments, answering the telephone and accurate message taking and distribution, facilitating the registration process, filing, documenting and maintaining a variety of statistical information, data entry and writing reports, preparation and maintenance of records, brochures, pamphlets, ordering office supplies and performing other duties as assigned.
QUALIFICATIONS :
- Community College diploma in Social Sciences (e.g. Human Services, Social Services Worker) and / or equivalent combination of education and experience within a healthcare setting;
- Significant experience related to HIV, Hepatitis C, infectious diseases, and working with vulnerable populations;
- Plexus certification;
- Knowledge and understanding of Ontario Trillium Drug Program, Ontario Disability Support Program and Ontario Works; Interim Federal Health, MORECARE, UHIP, Military, local sexual health clinics, uninsured care programs;
- Experience liaising with pharmacy representatives and data collection related to clinical trials;
- Intermediate level of computer skills, utilizing Microsoft Word, Excel and Outlook(tested), EPIC, OCEAN, MS Teams;
- Proven organizational, interpersonal and problem solving skills;
- Demonstrated knowledge and ability to work effectively with other health care providers, community support system / services, and social agencies;
- Sound knowledge of ethical and legal issues affecting people living with HIV, Hepatitis C
- Strong knowledge base understanding the social, psychological, cognitive, emotional and spiritual aspects (Biopsychosocial) as it relates to HIV / Hepatitis C patients and other vulnerable
populations
