Transition Social Worker (Children ➜ Adults) – Learning Disability & Mental Health

Long-term temp assignment

• 2 years
• 36 hrs pw
• Monday–Friday (occasional Sat)
• Wilshaw House, Tameside

Shape life-changing milestones – guide 17- to 25-year-olds as they move from Children’s Services into Adult LD & MH support.

What you’ll do

Assess & plan : strengths-based Care Act assessments, Mental Capacity decisions and outcome-focused support plans.

Coordinate care : commission services, monitor delivery and review changing needs for young people with learning disabilities, autism and / or mental-health conditions.

Collaborate : work hand-in-hand with CAMHS, SEND, schools, health and housing so every transition feels seamless.

Safeguard & evidence : prepare court statements (DoLS, COP DOL, s21a) and give confident evidence when required.

Champion carers : complete statutory Carer assessments and link families to the right support.

What you’ll bring

Social Work degree / DipSW & Social Work England registration.

Proven practice with young people who have learning disabilities and / or mental-health needs —ideally around transition.

Up-to-date knowledge of the Care Act, MCA & SEND / EHCP frameworks.

Strong assessment, report-writing and partnership skills, plus a full UK driving licence & car.

You must have :

A professional qualification in social work

Social Work England registration.

Post qualified experience.

Enhanced DBS / Update service.

References required for the last 5 years of employment.
