About the Role

Ortus Health is looking for a dedicated and experienced Remote Clinical Pharmacist to join our innovative healthcare team. In this fully remote role, you will be instrumental in providing high-quality pharmaceutical care and support to patients within a collaborative primary care model. You will have the opportunity to use your clinical expertise to enhance medication management, improve patient outcomes, and ensure safe prescribing practices.

As a Remote Clinical Pharmacist, you will conduct medication reviews, provide clinical consultations, and work closely with GPs and other healthcare professionals to optimise therapeutic outcomes for patients. This role is ideal for a proactive pharmacist looking to make a significant impact in a supportive environment.

Requirements

Key Responsibilities

  • Conduct comprehensive medication reviews for patients, focusing on safety, efficacy, and adherence.
  • Provide clinical advice on medication therapy management to both patients and healthcare providers.
  • Collaborate with multi-disciplinary teams to ensure optimal patient care and therapeutic outcomes.
  • Monitor and evaluate patient responses to medications and recommend adjustments to therapy as needed.
  • Stay updated with current pharmaceutical and clinical guidelines and contribute to the development of practice protocols.
  • Participate in continuous professional development and training opportunities.

Qualifications

  • Registered pharmacist with current GPhC registration.
  • Experience working within a clinical setting, preferably in primary care.
  • Strong knowledge of pharmacotherapy, chronic disease management, and public health.
  • Excellent communication skills for effective patient interaction and collaborative working.
  • Ability to work independently in a remote environment while demonstrating initiative and problem-solving skills.
  • A commitment to improving healthcare delivery and patient outcomes.
