Job description

GENERAL SUMMARY :

Provide individual tracking and monitoring services both through face-to-face monitoring and with the Global Positioning System (GPS) to youth and families through a multi-agency community based collaborative service delivery system. Tracking of clients occurs throughout Milwaukee County at their homes, schools, jobs or community etc. Monitoring services occur 24 / 7 for all required and assigned youth to the program .

ESSENTIAL FUNCTIONS :

  • Conduct face to face contacts at various locations with youth.
  • Develop and maintain helping relationships with youth in program to encourage individual accountability.
  • Complete all required documentation in an accurate and timely manner.
  • Enter data promptly in Agency and GPS tracking databases.
  • Review case files to become familiar with youth history.
  • Maintain all required trainings and certifications.
  • Adheres to St. Charles Youth & Family Services Policies and Procedures.
  • Adhere to the mandatory tracking hours as designated by program guidelines for all assigned cases.
  • Sustain timely responses to all inquiries by youth, families, Service Providers and Milwaukee County Department of Youth & Family Services.
  • Monitor youth via phone, GPS monitoring or through collateral contacts (other professionals, etc.). Monitor will need to be able to respond to client locations to address any concerns with 2

shift needs.

  • Supports the goals of the agency mission / vision.
  • Other duties as assigned.
  • MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED :

  • High School diploma or equivalent; Associate’s degree in Human Services, Criminal Justice or related field preferred
  • One year experience with programs that serve targeted population
  • Knowledge base of adolescent and family issues and dynamics.
  • Familiarity with database systems with ability to enter data frequently and timely.
  • Knowledge of Milwaukee County (Department of Human Services, Children’s Court, Milwaukee Public Schools, and Milwaukee community based youth serving agencies.)
  • Knowledge of human growth and development theories.
  • Ability to prepare formal reports as requested
  • Ability to present case in court if needed.
  • Ability to document all interactions and activities in a timely manner.
  • Ability to display cultural competence by responding respectfully and effectively to people of all cultures, languages, classes, races, ethnic backgrounds, religions and other diversity factors in a manner that recognizes, affirms and values the worth of individuals, families and communities and protects and preserves the dignity of each.
  • Valid Wisconsin driver’s license and liability insurance.
  • Ability to use vehicles including own for work related duties according to agency’s insurance liability standards.
  • Ability to accurately perform record keeping and documentation entries.
  • Ability to perform tasks using MS Windows applications and agency database systems.
  • MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED :

  • Indoor / outdoor requirements
  • Evenings and weekend requirements
  • Potential for physical intervention of clients as needed.
  • Exposure to lift a maximum of 50 lbs.
  • Local travel as required fulfilling essential functions of the position.
