Social Worker – Transfer of Care Team
Location : Cambridgeshire, United Kingdom
Rate : £38 per hour
An exciting opportunity has become available at Cambridgeshire for a Social Worker in the Transfer of Care Team.
Responsibilities include :
- Assessing individuals in hospitals for care packages or care home placements under the Care Act
- Conducting Mental Capacity Assessments for care and support
- Collaborating with multidisciplinary teams to create care plans
- Engaging in carers conversations and facilitating safeguarding discussions
