Job Title : Qualified Social Worker – Adults Hospital Team

Location : Stoke on Trent

Rate : Up to £30 per hour

An exciting opportunity has arisen for a Qualified Social Worker to join the Adults Hospital Team. This role is perfect for a dedicated professional looking to make a significant impact on the lives of adults in a hospital setting. You will be part of a supportive team, working collaboratively to ensure the best outcomes for patients and their families.

What will your responsibilities be?

As a Qualified Social Worker in the Adults Hospital Team, you will be responsible for undertaking complex assessments with service users accessing community hospital beds, including Care Act assessments and mental capacity assessments. You will develop and implement individualized care plans, provide emotional support and practical assistance, and coordinate with interdisciplinary teams to ensure seamless transitions. Additionally, you will advocate for patients’ rights, connect them with community resources, and maintain accurate case records. Your ability to work in a supportive and dynamic environment will be essential in ensuring the well-being and recovery of patients.

Qualifications and Experiences :

You must be registered with Social Work England and eligible to work in the UK. A full UK driving license is required, along with post-qualification experience. Additionally, you should hold a degree level qualification or equivalent in Social Work.
