We are looking foran experienced, enthusiastic, friendly and professional Specialist Diabetes Practice Nurse to join our team atPontesbury & Worthen Medical Practice on a permanent contract basis. Applicants must have experience of working in a GP Practice.
The successfulapplicant would be working alongside our current nursing team, and be responsible for the deliveryof Diabetes practice nursing services to our patient population.
Main duties of the job
Your main responsibilities will be the routine evidence based care of our diabetic population, including annual reviews incorporating the local prescribing guidelines as well as following up patients with more complex diabetic needs. The ideal candidate would have current Primary Care experience and would be a non-medical prescriber but this is not essential.

The role of the diabetes nurse is to promote the health and wellbeing of people with diabetes, to encourage compliance with medication through informed self-management.
Identify clinical risks and take appropriate action.
Provide a pathway of diabetes nursing care for the patient, referring to secondary care, multidisciplinary teams and voluntary services as required.
