Your Role And Responsibilities
As a Reablement Support Worker Team Leader, you’ll be at the heart of our mission to help people regain independence and confidence in their own homes.
You’ll lead a team of Reablement Support Workers, ensuring high-quality, person-centred care is delivered in line with each customer’s Short Term Reablement Plan (STEP). You’ll work closely with colleagues across health and social care to make sure services are responsive, effective, and empowering.

The team
Home First provides short-term reablement care and support to people in their own homes—often following a hospital stay, illness, or crisis. Our goal is to help people regain independence and avoid unnecessary hospital admissions.
You’ll be part of a passionate, collaborative team that works closely with Adult Social Care Cluster Teams, Independent Providers, Health colleagues, Borough and District Councils, the Multi-Agency Safeguarding Hub, and voluntary sector partners.
