Main duties of the job

Your new role

We are offering Clinical Nurse Specialist (CNS) or Clinical Paramedic Specialist (CPS) roles, equivalent to Band 7, within our Hospice Outreach Service in the community. These roles are part of our model to ensure we have the right people providing the right care, in the right place, at the right time.

The Hospice Outreach Team is the initial contact point for community referrals and incoming calls, providing palliative advice and support to patients and families at home, including care and nursing homes. You will join a rota covering our seven-day service, offering telephone support, clinic, and home assessments.

Our team cares for patients with life-limiting illnesses, focusing on individualised care to improve quality of life and achieve patient-centred goals. We care for those with long-term conditions such as heart failure, respiratory diseases, frailty, neurological conditions, and oncological diagnoses.

At the Hospice, we collaborate with other community teams and specialists to optimise patient experience and team development.

About us

To succeed in this role...

You are a motivated clinician capable of working independently and as part of a responsive team. You possess advanced communication and presentation skills (oral and written).

You have proven community or inpatient palliative care experience at Band 7 or equivalent in related areas such as Paramedic, General Medicine, Neurology, Emergency Medicine, Frailty, Respiratory Care, Cardiology, or Oncology. Sound pharmacological knowledge of palliative / end-of-life medications is essential.

Job Title : Clinical Nurse Specialist (CNS) / Clinical Paramedic Specialist (CPS)

Location : Based at Pembury or Five Ashes, with potential to work at other locations within West Kent and East Sussex.

Scope & Job Purpose : Coordinate and deliver high-quality care to patients with terminal illnesses, providing support and advice in their homes, community hospitals, and care / nursing homes. Support the delivery of hospice advice and support, provide clinical advice to professionals, and help patients achieve personal goals and preferred place of care.

Key Responsibilities :

• Manage patients effectively, ensuring appropriate care in the right setting at the right time.
• Work as part of a team to meet referral outcome measures.
• Attend palliative care meetings at GP practices.
• Promote End of Life Care tools.
• Provide tailored information to patients and families about their condition, treatment, and services.
• Perform comprehensive physical assessments holistically.
• Ensure symptom control and End of Life Care are delivered, documented, and reviewed to high standards.
• Review patients at MDT or caseload meetings following NICE guidelines.
• Participate in a 7-day service, including weekends and bank holidays, with duty planning.
