Were proud to be recruiting for a Service Coordinator to join our dedicated team. As a Service Coordinator, you will be working to deliver high-quality support. Youll support customers who have complex needs and who are experiencing multiple disadvantages, including substance use, mental health, offending, and rough sleeping / homelessness. Reporting to the Support Team Leader, you will coordinate customer access to a wide range of services, and you will lead the design and delivery of a service coordination plan alongside the customer and other essential agencies.

Job requirements

Provide flexible, compassionate support to customers with complex needs, including homelessness, substance use, mental health.

Empower customers to regain independence and help them understand what it takes to manage their own tenancy.

Work closely with the multi-agency team and support network to strengthen support.

Develop and design tailored service / support plans.
