Job Title : Social Worker - Adult Reablement - Stockport

Location : Stockport (Hybrid)

Pay Rate : 30 per hour

About the Role :

Liquid Personnel is seeking an Social Worker to join an esteemed clients Adult Reablement Team.

Responsibilities :

In this role you will

  • Hold a short-term caseload within a reablement service monitoring progress completing Care Act assessments and responding to safeguarding concerns.
  • Carry out formal mental capacity assessments prioritise and review cases and offer information advice and signposting where appropriate.
  • Work in a strengths-based person-centred way with individuals living with varying levels of risk using creativity and flexibility to support outcomes.
  • Contribute social work expertise within a small supportive team and wider multi-disciplinary service supporting colleagues apprentices and shadowing.
  • Attend team meetings and training adhere to council policies and adult legislation and work flexibly across home and office settings (car driver essential).
