GP Clinical Pharmacist

Queens Crescent and Hyndland Road Surgery team, is looking to recruit a motivated, proactive and enthusiastic pharmacist to work within our GP practices. This role will take a leading role in medicines management and medication review as part of the primary care multidisciplinary team. It is a rewarding role, helping patients to safely manage their medicines.

You will provide a clinical pharmacy service directly to patients. Day-to-day responsibilities might include : clinical assessment and review of patients, performing complex polypharmacy medication reviews with patients, dealing with medicines related queries, assessing patients ability to manage their medicines (including in the patients home), medicine reconciliation and prescribing of medication. Joint working with GP practice staff will be routine, as well as with the multidisciplinary teams within the local hospitals, Community Pharmacies and Nursing homes.
