Your role and responsibilities

As a Reablement Support Worker Team Leader youll be at the heart of our mission to help people regain independence and confidence in their own homes.

Youll lead a team of Reablement Support Workers ensuring high-quality person-centred care is delivered in line with each customers Short Term Reablement Plan (STEP). Youll work closely with colleagues across health and social care to make sure services are responsive effective and empowering.

On a typical day you might :

  • Develop and review reablement plans with customers adjusting them as needed to meet agreed outcomes.
  • Supervise and support your team providing professional guidance and performance feedback.
  • Ensure compliance with health and safety policies including risk assessments and manual handling.
  • Collaborate with Integrated Neighbourhood Teams hospital teams and voluntary sector partners to deliver joined-up care.
  • Contribute to service development and participate in HR processes such as Occupational Health referrals and performance management.
  • You will need

    We hope youll bring :

  • A good level of education (e.g. NVQ Level 3 in Care or Health) or equivalent experience with a willingness to work towards further qualifications.
  • Supervisory experience or a relevant qualification such as ILM (desirable).
  • Strong leadership skills to manage a dispersed team effectively.
  • A good understanding of the care needs of different service user groups including older people and those with physical or sensory disabilities.
  • Confidence in assessing staff performance and making informed decisions.
  • A proactive enabling approach that supports people to achieve their goals.

    The team

    Home First provides short-term reablement care and support to people in their own homesoften following a hospital stay illness or crisis. Our goal is to help people regain independence and avoid unnecessary hospital admissions.

    Youll be part of a passionate collaborative team that works closely with Adult Social Care Cluster Teams Independent Providers Health colleagues Borough and District Councils the Multi-Agency Safeguarding Hub and voluntary sector partners.
